are $199, $275, and $375 respectively, including one $100 household registration fee. Add a child to
Individual or Couple for $50/month. The Family Core Care Plan includes up to 2 adults and 2 children; each additional child is $30/month.

No. Direct Primary Care is a direct-pay membership model. It is not health
insurance and does not replace coverage for emergency care, hospital services, surgery, imaging,
specialist services, or other care outside the membership scope.

Yes. White Tank Wellness is located in Buckeye and serves in-person patients
from Buckeye, Verrado, Goodyear, Surprise, Avondale, Litchfield Park, and nearby West Valley
communities. The practice also serves patients throughout Arizona with virtual visits when clinically
appropriate.

No. Labs are not required to enroll in Direct Primary Care. If your provider orders testing during care, separate laboratory charges may apply. BHRT and Complete Care require labs for each adult receiving hormone care; children are not eligible for BHRT.
Complete Care combines DPC and BHRT for $199/month for one adult or $350/month for a couple. Active DPC members do not pay the $100 DPC registration fee again; applicable hormone-care startup and adult lab charges still apply. Existing BHRT members can add DPC without an additional startup fee. The clinic confirms upgrade charges and billing timing. Medical weight loss is separate.
